1. THE REVIEW OF 85 CASES WITH HEPATIC HYDATIC CYSTS

In this paper, 85 patients with hepatic hydatic cysts, who were treated surgically and whose diagnoses were confirmed by pathological analysis between 1988-1993 in the 1. and 2. Surgery Clinics of Kartal Education and Research Hospital are demonstrated. We found that 55.2% of these patients were from the East Anatolia and 57.6% of them developed complications. The most frequent complication was secondary bacterial infection (25.8%). The reason why our complication rate was so high is the delayed application of our patients. We revealed that ultrasonography was the first diagnostic tool with 94.9% accuracy. Most frequently applied surgical procedure was external tube drainage (31.7%). Despite its high morbidity and prolonged hospital stay, this is still the most effective and simple procedure of treatment in superinfected hydatic cyst cases.

We conducted a detailed clinical and laboratory intervention to the patient who came to our emergency unit in deep coma and died in 15 hours. At the end of histopathological approach by needle biopsy, we concluded that she developed a Reye-like syndrome probably due to her metabolic deficiency and advised her family to ask for genetic counseling at a university clinic. By means of this patient, we reviewed Reye and Reye-like syndromes up to date.

3. PANCREATICODUODENECTOMY PERFORMED FOR PANCREATICO-DUODENAL INJURY

Combined pancreas and duodenum injuries are the most serious lesions in the gastrointestinal system. If injuries involve duodenum, ampulla Vateri and head of the pancreas all together, pancreaticoduodenectomy can be performed. In this report, we discuss the case in the view of the literature and suggest that, if performed carefully, complications of pancreaticoduodenectomy would be minimal and the operation be well-tolerated by the patient.

4. BILATERAL LIPID CELL TUMOUR

A 65-year-old female was admitted to hospital with abdominal pain and mass. On CT examination a right ovarian mass was found. Total hysterectomia and bilateral salpingooopherectomia was performed. In both ovaries, though larger in the right one, there were yellow, soft, sharply circumscribed tumors. On HE, the tumor had big, polygonal cells with abundant granular cytoplasm. The cells formed sheets and cords separated by a vascular network. On neutral lipid dyes, tumor cell cytoplasm contained lipid. Reinke crystalloids were not found. There was no androgenic activity on clinical and laboratory examination.

5. A CASE OF ANDROGENINSENSITIVITY SYNDROME WITH GONADAL ANDROGEN RECEPTOR POSITIVE

Androgen insensitivity syndrome is a rare endocronogical disease. Our case was 17 year old, genotypically male, fenotypically female. Her clinical and laboratory features were parallel to the testicular feminization syndrome, however, being receptor positive. It was considered to be a variant of testicular feminazition syndrome with a structural abnormality of receptor protein or a disorder in postreceptor mechanism.

Intraabdominal organ and tissue gunshot wounds are commonly encountered. Small caliber bullets travelling at high speed can destroy a large amount of tissue because of their great energy. After such injuries a retroperitoneal hematoma may be a sign of lacerations at large vessels. We present a case report about injuries at common iliac vein and superior mesenteric artery due to a gunshot wound.
