Anesthetıc Approach Of Prımary Hypothyroıdısm: Case Report

Hypothyroidism may lead to depression of the cardiovascular system, abnormal baroreceptor dysfunction, decrease in plasma volume, dysfunction of hepatic drug metabolism, hypoglycemia, hyponatremia, and hypothermia. As such cases present increased sensitivity to inhalation anesthetics, they should be treated until they reach an euthyroid state. Anesthesiologists face hypothyroidism in many patients who are in need of urgent surgery. In this case report, we present the anesthetic management of a female patient aged 69 who had to undergo an urgent partial hip arthroplasty and was diagnosed with hypothyroidism. We discuss the case in the light of the current literature.

Hipotiroidizm, kardiyovasküler sistemde depresyon, anormal baroreseptör fonksiyonu, plazma volümünde azalma, karaciğer ilaç metabolizmasında bozulma, hipoglisemi, hiponatremi ve hipotermiye neden olabilir. Bu tür olgularda genel anesteziklere karşı artmış duyarlılık olduğundan, hasta cerrahi öncesi mutlaka ötiroid duruma getirilmelidir; ancak anestezistler acil ameliyat gerektiren bazı hastalarda hipotiroidi ile karşılaşmaktadırlar. Bu olgu sunumunda, ameliyat öncesi değerlendirmede hipotiroidi tanısını konulan ve acil parsiyel kalça protezi uygulanan 69 yaşındaki kadın olgunun anestezi uygulaması literatür eşliğinde sunuldu.
