Hiperbilirubinemi ve sepsis ön tanısı ile takip edilirken exitus olan altı günlük kız bebek. Aile anamnezinde, iki kardeş (15 aylık ve 20 günlük iken) ateş ve sanlık bulguları ile kaybedilmiştir. Otopside; yaygın ikterik görünüm, akciğerlerde solid, parlak pembe, kanamalı alanlar, karaciğer ve timusta san renkli görünüm mevcuttu. Mikroskopide; nekrotizan pnömoni, karaciğerde sekonder kolestaz, timusta involusyon ve lenfosit kaybı saptandı. İmmun yetmezlik zemininde, akciğerlerde fulminant invaziv aspergillosis (İA) olgularında etyolojik faktörlerin varlığı için klinik ve laboratuar çalışmaların derinleştirilmesinin önemini vurgulamak istedik.
We report a six-day-old girl newborn who died during monitoring with an initial diagnosis of hyperbilirubinemia and sepsis. Family history revealed that another two siblings died with manifestations of fever and jaundice at the age of 15 months and 20 days, respectively. Autopsy examination showed widespread icteric appearance, solid, bright pink, and hemorrhagic areas in the lungs, and yellowish appearance in the liver and thymus. Microscopic study showed necrotizing pneumonia, cholestasis in the liver, and involution and loss of lymphocytes in the thymus. We emphasize the need for further clinical and laboratory studies to seek for etiologic factors in cases of fulminant invasive aspergillosis in the lungs developing on the basis of immune deficiency.
