ON YILLIK DOĞUM TRAVMASI OLGULARININ DÖKÜMÜ

Son 10 içinde (1981 - 1990) Şişli Eftal Hastanesi II. Çocuk Kliniğine 217 doğum travması olgusu yatmıştır. Bu servise yatan tüm hastaların %1.99'unu oluşturmaktadır ve yıllara göre %0.7-3 arasında değişmektedir. Erkek/Kız oram l/l.92 olarak saptandı, ayrıca doğum ağırlığının, prezantasyon bozukluklarının, müdahaleli doğumların ve annenin gebelik sayısının doğum travmasına hazırlayıcı risk faktörleri olduğu açıklandı. Doğum travmasına bağlı ölüm oranı %36 olarak tespit edildi. Bunun büyük çoğunluğunu beyin travmaları oluşturmaktadır ve yaşayan çocuklarda da tedavisi mümkün olmayan sekeller bırakmaktadır. Bundan dolayı doğum travmasının tedavisinden çok oluşmasının önlenmesi gerektiğini vurguluyorum. Burada doğum yaptıracak hekimlere, pediatristlere, nöropediatristlere ve diğer sağlık elemanlarına önemli görevler düşmektedir.

THE SCREENING OF BIRTH INJURIES OF THE LAST TEN YEARS

In the last ten years (1981 -1990), 217 newborn with birth traumas had been accepted in to the II.. Pediatric clinic of Şişli Etfal Hospital. This is 1.99% of the total patients hospitalised by that department and this percentage varies between 0.7-3% yearly. The ratio of boys to girls is 1/1.92. Birth weight, malpresantation, complicated births and the parite are accepted to be the risc factors for birth traumas. The mortality rate of birth traumas was found as 36%. Cerebral traumas are responsible for the majority of mortalities or causes mostly results with heavy sequeles. For this reason, we emphasised the importance of prophylaxis of birth traumas rather than therapy. The role of the team work between the obstetricians, neonatologists, neureopediatrists and other health care personnel is important for prophylaxis.
