THE COMPARISON OF INTRACAVITARY INSTILLATIONS OF BCG AND MITOMYCIN-C IN SUPERFICIAL BLADDER CARCINOMAS

In this prospective randomized study, efficacy of the intravesical BCG and Mitomycin-C treatment after transurethral resection of superficial bladder cell cancers were analyzed. In our clinic between June 1996 and December 1999, 47 patients with the diagnosis of superficial bladder cell cancer stage Ta, T1 were included in this study. There were 22 patients in BCG group and 25 in Mitomycin-C group. There was no statistical difference according to the age, sex and tumour characteristics between the two groups. After two weeks of transurethral resection, 81 mg. BCG or 40 mg. Mitomycin-C were given transurethrally once a week for 6 weeks according to the their groups included. Mean follow up was 21,3 months. During follow up period, systoscopy and random mucosal biopsy were performed in every 3 months. In cases detected tumour cystoscopically and/or pathologically were accepted as recurrence. Tumour recurrence rate was 27,3% for BCG group and 32% for Mitomycin-C group (p>0.05). Mean time from resection to recurrence was 8,0±1,5 months for BCG group and 7,3±1,2 months for Mitomycin-C group (p>0.05). Irritable bladder symptoms were observed in some of the BCG group patients, while the other group has no side effects. As a result, efficacy of the BCG and Mitomycin-C treatments were not different statistically in our study. BCG group have more side effects than Mitomycin-C group. Higher cost of Mitomycin-C may be a disadvantage. Both drugs have same efficacy and may be preferred for the treatment of superficial bladder cell cancers.

YÜZEYEL MESANE TÜMÖRLERİNDE İNTRAKAVİTER BCG VE MİTOMYCİN-C ETKİNLİĞİNİN KARŞILAŞTIRILMASI

Bu prospektif randomize çalışmada, yüzeyel değişici epitel hücreli mesane kanserlerinde, transüretral rezeksiyon sonrası profilaktik intravezikal BCG ve Mitomycin-C tedavi etkinliğinin karşılaştırılması amaçlandı. Haziran 1996-Aralık 1999 tarihleri arasında kliniğimizde stage Ta, T1 yüzeyel değişici epitel hücreli mesane kanseri tanısı konulan 47 olgu çalışmaya alındı. BCG grubunda 22, Mitomycin-C grubunda ise 25 olgu vardı. Yaş, cinsiyet ve tümör özellikleri yönünden iki grup arasında anlamlı bir fark yoktu. Transüretral rezeksiyonu izleyen ikinci haftadan itibaren 6 hafta boyunca haftada bir, BCG grubuna 81mg BCG, Mitomycın-C grubuna ise 40mg Mitomycin-C intravezikal uygulandı. Olgular üç ayda bir sistoskopi ve random mukozal biopsi yapılarak ortalama 21.3 ay izlendiler. Sistoskopik ve/veya patolojik olarak tümör gözlenen olgular rekürrens kabul edildiler. Takipte BCG grubunda %27.3, Mitomycin-C grubunda ise %32 tümör rekürrensi gözlendi (p>0.05). Rekürrense kadar geçen ortalama süre sırasıyla 8.0±1.5 ay ile 7.3±1.2 ay idi (p>0.05). BCG grubundaki olguların bir kısmında irritabl mesane semptomları görülürken, Mitomycin-C grubundaki olgularda ciddi bir yan etki gözlenmedi. Sonuç olarak, yaptığımız çalışmada BCG ve Mitomycin-C grupları arasında etkinlik açısından fark saptanmamış olup BCG uygulamasında daha fazla yan etki görülürken, maliyet açısından Mitomycin-C'nin dezavantajlı olduğu saptanmıştır. Yüzeyel değişici epitel kanseri tedavisinde her iki ilaç da benzer etkiye sahip olduğundan tercih edilebilir.
