AN UNUSUAL CAUSE OF ANTERIOR KNEE PAIN: IDIOPATHIC PATELLA OSTEONECROSIS

A 40 years old woman who had a history of right knee pain for 2 years applied to our hospital. There was no abnormality with her biochemical laboratory tests, and recall no trauma. Examination of the knee was unremarkable except for slight pain over the superolateral corner of the patella. Radionucleid bone scan showed increased uptake in the patella, MRI showed grade 4 chandromalacia of superolateral side of the patella. Arthroscopy revealed chondromalacia at the superolateral aspect of the patella of the right knee, by the way biopsy was performed from the injured aspect, and decompression was made. After 2 years follow up, the patient had no compliants and examination of the knee was normal. Osteonecrosis of the patella was a rare case and had been reported after trauma, use of steriods, alcohols, hormonal and hemotological diseases, rarely my be idiopathic. The onset of idiopathic Osteonecrosis of the patella was knee pain and the diagnosis was made by radioisotopic bone scan, MRI, histopathological examination. Idiopathic Osteonecrosis of the patella should be considered as a possible cause of anterior knee pain.

DİZÖNÜ AĞRISININ NADİR NEDENLERİNDEN BİRİ: İDYOPATİK PATELLA OSTEONEKROZU

Kırk yaşında bayan hasta. İki yıllık diz ağrısı şikayeti ile başvurdu. Anamnezi ve biyokimyasal tetkikleri normaldi. Fizik muayenesinde patella superolateral köşesi kompresyonla ağrılı bulundu. Sintigrafide "uptake" artışı, MRG'de patellada evre 4 yumuşama saptandı. Yapılan diagnostik artroskopide, patella superolateral köşede kondral yumuşama saptanıp, aynı sahadan osteokondral biyopsi alınarak dekompresyon yapıldı. Biyopsi sonucunda patellada osteonekroz saptandı. Postoperatif ikinci yıl kontrolünde şikayetleri olmayan hastanın fizik muayenesi normaldi. Patella osteonekrozu nadir bir hastalık olup, etyolojisinde daha çok travma, steroid kullanımı, alkol kullanımı, hormonal ve hematolojik hastalıklar rol alır. Nadir olarak idyopatik oluşabilir. İdyopatik patella osteonekrozu karşımıza diz ağrısıyla çıkar ve kesin tanısı kemik sintigrafisi, MRG ve histopatolojik incelemeyle konur. Dizönü ağrısıyla başvuran hastalarda patella osteonekrozu ayrıca tanıda göz önünde bulundurulmalıdır.
