OBJECTIVE: Phytobezoar is a gastric concretion composed of vegetable matter found in the alimentary tract. It is relatively uncommon cause of small bowel obstruction and is often associated with a history of recent pulpy foods such as persimmons and oranges. In the development of nonpersimmon phytobezoar the key element is gastric stasis, often induced by gastric surgery. Although the treatment of choice for gastric phytobezoar is nonoperative, based on gastric lavage, clear fluid diet or endoscopic fragmentation and removing, phytobezoars presenting as acute intestinal obstruction require mandatory operative management. To review the diagnostic and therapeutic implications of small bowel obstruction secondary to phytobezoars, we retrospectively evaluated seven patients with intestinal obstruction due to phytobezoar that we operated.

Fitobezoar Nedeniyle Oluşan Mekanik Bağırsak Tıkanıklığı Olguları*

AMAÇ: Fitobezoar, gastrointestinal sistemde bulunan gıdaların sertleşmesi ile oluşur. Bunlar ince bağırsak tıkanıklıklarının nispeten nadir nedenlerindendir. Genellikle portakal, Trabzon hurması gibi yumuşak, lifli gıda alınımı anamnezi ile birliktedir. Yumuşak ve liflilerin dışındaki gıdalarla fitobezoar gelişmesinde temel neden sıklıkla mide ameliyatlarının yol açtığı gastrik stazdır. Mide fitobezoarlarının seçilecek tedavisi gastrik lavaj, berrak sıvı diyet veya endoskopik parçalama ve çıkarma gibi ameliyatsız yöntemler olmakla birlikte, akut intestinal tıkanmayla kendini gösteren fitobezoarlarda kesinlikle cerrahi tedavi gerekir. Bu yazıda fitobezoarlara sekonder gelişen ince bağırsak tıkanıklıklarında tanı ve tedavi ile ilişkili faktörleri gözden geçirmek amacıyla, fitobezoar nedeniyle oluşan yedi intestinal tıkanıklık olgusu retrospektif olarak değerlendirildi.
