Modified Bilhaut-Cloquet procedure for Wassel type III polydactyly of the thumb

Thumb duplication is a pre-axial polydactyly and one of the most frequent congenital deformities of the hand after syndactyly, and the incidence of preaxial polydactyly is reported as 1 in every 3.000 live births. The surgical options for congenital thumb duplication fall into two groups: ablation of one of the duplicated thumbs, with or without reconstruction of the other part, and the Bilhaut-Cloquet procedure or a modification of it. In this case study, our aim was to emphasize the effectiveness of the modified Bilhaut-Cloquet procedure for preserving the metacarpophalangeal joint motion and proximal phalanx unity in more deformed Wassel type 3 polydactyly.

Polidaktili sindaktiliden sonra üst ekstremitenin en sık görülen doğumsal anomalisidir. Preaksiyel (radial) polidaktili en sık görülen alt grubudur ve her 3000 canlı doğumda bir görüldüğü bildirilmiştir. Radial polidaktili tedavisinde temel olarak iki cerrahi yaklaşım vardır: Biri duplike başparmaklardan birinin ablasyonu ve gerekirse diğer parmağın rekonstrüksiyonu, diğeri ise Bilhaut-Cloquet yöntemi veya modifikasyonlarıdır. Bu olgu sunumundaki amacımız aşırı deforme Wassel tip 3 polidaktilide metakarpofalangeal eklem hareketini ve proksimal falanksı korumak ve güçlendirmek için modifiye Bilhaut-Cloquet yönteminin uygun bir tercih olduğunu vurgulamaktır.
