The Early Results of Treatment With Double Button Technique in Acute Rockwood Type III-V Injuries of Acromioclavicular Joint Injuries

INTRODUCTION: The aim of this study was to evaluate the preliminary results of acute acromioclavicular (AC) joint injuries with open double button technique.
METHODS: Twelve patients (10 men, 2 women) treated with double button technique in AC joint injuries between August 2009 and April 2013 were included in the study. The mean age was 32 (range: 21–51) years. Seven of the patients had type III, 2 had type IV, and 3 had type V injuries according to the Rockwood classification. TightRope® (Arthrex, Naples, Florida, USA) system was used to fix the AC joint. The mean surgical time was recorded. Visual analog score (VAS) and Constant shoulder score (CSS) was used in functional evaluation. Coracoclavicular (CC) distance was measured in radiographic evaluation.
RESULTS: The average follow-up was 14.5 (range: 3–38) months. At the most-recent follow-up, 10 patients were exemplary, while the remaining 2 patients showed satisfactory functional results. The mean VAS score improved from 7.4±0.9 preoperatively to 2.1±0.9 postoperatively. The mean CSS improved from 35.6±8.0 preoperatively to 81.9±8.8 postoperatively. These improvements were statistically significant (p<0.05). The CC distance of the operated and unoperated sides of the shoulders was not statistically significant (p=0.413).
DISCUSSION AND CONCLUSION: The double button technique is straightforward, has low complication rates, offers high stability, and provides minimally invasive surgery in acute AC joint injuries. Due to these advantages, it is the most-effective technique in AC joint injuries, and therefore should be used as the primary technique in such case.

Akromiyoklaviküler Eklemin Akut Rockwood Tip III-V Yaralanmalarının Çift Düğme (Button) Yöntemi ile Tedavisi: Erken Dönem Klinik ve Radyolojik Sonuçlarımız

GİRİŞ ve AMAÇ: Çalışmamızda akut akromiyoklaviküler (AK) eklem yaralanmasında uyguladığımız açık çift düğme (button) tekniğinin kısa dönem klinik ve radyolojik sonuçları değerlendirildi.
YÖNTEM ve GEREÇLER: Ağustos 2009–Nisan 2013 tarihleri arası AK eklem hasarı nedeni ile çift düğme tespit yöntemi uygulanan 12 hasta (10 erkek, 2 kadın) çalışmaya alındı. Ortalama yaş 32 (dağılım, 21–51 yaş) idi. Olguların yedisinde Rockwood tip III, ikisinde tip IV, üçünde tip V AK eklem hasarı tespit edildi. Akromiyoklaviküler eklem tespiti için TightRope® sistemi kullanıldı. Ortalama ameliyat süreleri kayıt edildi. Fonksiyonel değerlendirmede Vizüel Analog Skala (VAS) ve Constant omuz skoru (COS) kullanıldı. Radyolojik olarak korakoklaviküler (KK) mesafe ölçüldü.
BULGULAR: Ortalama izlem süresi 14.5 (dağılım, 3–38) aydı. Son izlemde 10 hastada mükemmel ve iki hastada iyi fonksiyonel sonuç elde edildi. Ortalama VAS skoru ameliyat öncesi 7.4±0.9’dan son kontrolde 2.1±0.9’a geriledi. Ortalama Constant omuz skoru ameliyat öncesi 35.6±8.0’dan son kontrolde 81.9±8.8’e yükseldi. Hem VAS hem de COS değerlendirildiğinde düzelme anlamlı idi (p<0.05). Ameliyat sonrası etkilenmiş taraf ortalama KK mesafe normal omuza göre kıyaslandığında anlamlı bir fark yoktu (p=0.413).
TARTIŞMA ve SONUÇ: Çift düğme tekniği kolay, pratik, minimal invaziv, düşük komplikasyon oranı ve yüksek stabiliteye sahip bir yöntem olarak akut AK eklem yaralanması olan olgularda öncelikle seçilebilecek bir yöntemdir.
