Evaluation of Dexamethasone-Haloperidol and Dexamethasone-Metoclopramide for the Treatment of Post-operative Nausea and Vomiting

OBJECTIVE: Nausea and vomiting are one of the most common complications during the post-operative period. The aim of the present study was to evaluate anti-emetic efficiency using the combination of dexamethasone and other commonly used anti-emetic agents, haloperidol and metoclopramide.
METHODS: Group D was treated with 5 mg IV dexamethasone prior to the induction of anesthesia; Group DH was treated with 5 mg IV dexamethasone as a bolus prior to the induction of anesthesia and 2 mg IV haloperidol 15 minutes before extubation; Group DM was treated with 5 mg IV dexamethasone prior to the induction of anesthesia and 10 mg IV metoclopramide before extubation. Nausea-vomiting was assessed using the 5-point Verbal Descriptive Scale (VDS).
RESULTS: The evaluation of patients who experienced post-operative nausea and vomiting revealed significantly lower VDS scores in patients treated with DH or DM compared to patients who received the placebo (p<0.05).
CONCLUSION: DH and DM combinations were effective prophylaxis against postoperative nausea and vomiting. However, the highest VDS scores occurred in the DM group. As a result, we concluded that DM combination would be superior to DH combination for the prevention of postoperative nausea.

Deksametazon-Haloperidol ve Deksametazon-Metoklopramid Kombinasyonlarının Ameliyat Sonrası Bulantı ve Kusma Üzerine Olan Etkilerinin Karşılaştırılması

AMAÇ: Ameliyat sonrası dönemde en sık karşılaşılan bulantı ve kusma, ciddi bir sorun olmaya devam etmektedir. Antiemetik özelliği olan deksametazonu diğer sık kullanılan antiemetikler haloperidol ve metoklopramid ile kombine ederek etkinliklerini karşılaştırmayı amaçladık.
YÖNTEMLER: Ameliyat sonrası dönemde bulantı kusma gözlenen hasta verileri incelendiğinde; DH ve DM kombinasyonlarının kullanıldığı hasta gruplarında, plasebo kullanılan hasta gruplarına göre, istatistiksel olarak anlamlı ölçüde daha düşük VDS değerlerinin elde edildiği görülmüştür (p<0.05).
BULGULAR: Hastanemizde elektif koşullarda laparoskopik kolesistektomi operasyonu planlanan ASA I-II grubundan, 18-65 yaşları arasında 80 hasta çalışmaya dahil edildi. Grup D’ye anestezi indüksiyonu öncesi deksametazon 5 mg iv, Grup DH’ye anestezi indüksiyonu öncesi deksametazon 5 mg iv bolus, ekstübasyondan 15 dk önce haloperidol 2 mg iv, Grup DM’ye anestezi indüksiyonu öncesi deksametazon 5 mg iv, ekstübasyondan 15 dk önce metoklopramid 10 mg iv uygulandı. Olgulardaki bulantı ve kusma şiddeti beş aşamalı Verbal Deskriptif Skala (VDS) ile değerlendirildi. Bulantı-kusma skoru 5, 15, 30. dakikalarda ve 1, 3, 6, 24. saatlerde kaydedildi.
SONUÇ: Çalışmamızda deksametazonun 5 mg’lık dozunun ameliyat sonrası bulantı kusmayı önlemede yetersiz kaldığı, DH ve DM kombinasyonları kullanımının POBK profilaksisinde etkili olduğu görülmüştür. Ancak en iyi VDS değerinin DM grubunda görülmesi nedeniyle DM grubunun DH grubuna göre daha iyi bir tercih olacağı sonucuna vardık.
