The Effect Of Metronidazole And Intraperitoneal Lavage On The Complications In Secondary Peritonitis Cases

Postoperative intraabdominal abscess, brid ileus, evisceration and wound site infections constitute major problems for surgeons. It is known that antibiotics administered parenterally and enterally can reduce these complications. However, no study concerning the changes in complications and mortality upon intraabdominal use of antibiotics was performed in recent years. 92 consecutive patients operated in our clinic due to fecal or purulent peritonitis were categorized into two groups and observed. Preoperative intraabdominal metronidazole and lavage was applied in addition to systemic antibiotics to 46 patients in the study group. Complications were observed only in 2 patients (4%) within this group. These consisted of wound site infection and evisceration. On the contrary, 46 patients in the control group were received only systemic antibiotics, upon which wound site infection in 8 of patients (17%) and evisceration in 1 case (2%) occurred (p<0,05). Although 3 of the patients in the study group died (p<0,05), we think that rather than our study, these conditions were related to the primary pathologies of the relevant patients died. No difference concerning the other complications was observed between these two groups (for each p>0,05). In conclusion, administration of intraabdominal metronidazole, lavage and systemic antibiotic (combined therapy) may reduce the complications in fecal and purulent peritonitis patients

Sekonder Peritonitlerde Metronidazol İle İntraperitoneal Lavajın Komplikasyonlar Üzerine Olan Etkisi

Postoperatif intraabdominal abse, brid ileus, eviserasyon ve yara yeri enfeksiyonları cerrahlar için büyük problemler oluşturmaktadır. Parenteral ve enteral uygulanan antibiyotiklerin bu komplikasyonları azalttığı bilinmektedir. Ancak antibiyotiklerin intraperitoneal kullanımı ile komplikasyonların ve mortalitenin değişimi ile ilgili bir çalışma son yıllarda yapılmamıştır. Kliniğimizde fekal ya da pürülan peritonit sebebi ile opere edilen 92 ardışık hasta iki gruba ayrılarak incelenmiştir. Çalışma grubundaki 46 hastaya sistemik antibiyotiklerin yanı sıra peroperatif intraperitoneal metronidazol ile lavaj uygulanmıştır. Bu grupta sadece 2 (%4) hastada komplikasyon gözlenmiştir. Bunlar yara yeri enfeksiyonu ve eviserasyondur. Buna karşın kontrol grubundaki 46 hastaya sadece sistemik antibiyotik verilmiş olup, hastaların 8’inde (%17) yara yeri enfeksiyonu, 1 hastada (%2) eviserasyon oluşmuştur (p<0,05). Her ne kadar çalışma grubundaki 3 hasta kaybedilmiş olsa da (p<0,05), bu sonucun çalışmamızla ilişkili olmasından ziyade ölen hastaların primer patolojileri ile ilişkili olduğunu düşünmekteyiz. Her iki grup arasında diğer komplikasyonlar açısından bir farklılık görülmemiştir (her biri için p>0,05). Sonuç olarak intraabdominal metronidazol ile lavaj ve sistemik antibiyotik kullanımı (kombine tedavi) fekal ve pürülan peritonitli hastalarda komplikasyonları azaltabilir.
